Player Registration
|
Instructions for Player Registration:
- Login to the SLSL website. If you don't know your username and password, the login screen will give you options to retrieve your login information.
- If you are not in the SLSL database then you will need to have your information added to the database, click here to enter your information. Once you are entered into the database, your username and password will be emailed to you. Please allow 1-3 days for this process to take place.
- In order to register a player(s), you will need the following items:
- Parent/Guardian Information (Names, Address, Contact Numbers, Email Addresses)
- Player(s) (Name, Gender, Grade, Date of Birth)
- Insurance Information (Name and Policy Number)
- Doctor/Physician Practice (Name and Phone Number)
- Once you have completed the 0n-line registration process, you will be prompted to print out the registration forms. You will need to turn in these forms at registration (or mail in these forms) so if you want a duplicate copy for yourself, make sure you print these at the same time.
- Once the forms are printed, the following information must be brought to a SLSL Registration (see calendar for the date, time, and location of a SLSL Registration):
- Registration forms, signed by Parent/Guardian (blank registration forms will be provided at the SLSL Registration if you have not filled out the forms on-line and printed a copy).
- Payment matching the amount displayed on the registration forms.
- Shown below are the Sport Seasons that are open for registration.
- To continue with registration, click on the "Continue with Registration" button at the bottom of this page.
